下一场患者安全运动是可持续医疗照护。麻醉气体是造成医疗照护污染和排放的主要来源。这为麻醉医师提供了机会来极力减少医疗照护对群体健康的影响。如果我们能大幅减少本专业的排放,那么我们就可以向整个医疗照护部门证明,可持续健康照护是可能实现的,这对于我们患者的健康也是必要的。
医疗照护相关污染造成的伤害等同于引起患者安全运动的医疗差错。1 在医疗照护部门排放的温室气体 (GHG) 中,麻醉气体量最大,但其也是最易改变的排放源之一。2 麻醉专业人士有机会来领导医疗照护部门对抗污染和气候变化。
去年夏天,一名居住在大不列颠哥伦比亚省气候温和地区的老妇人,在经历了一波极端高温天气后出现了呼吸困难——经医生诊断,认为她患有“气候变化病”。3 尽管她是第一个被诊断出患有“气候变化病”的人,但她并不是第一个因为气候变化而出现健康问题的患者。4 她也不会是最后一个。
气候变化已经影响到了患者的健康。但是,医疗照护部门也逐渐认识到环境卫生对于群体健康的影响,并着手评估其对于全球排放和气候相关性死亡率的巨大影响。4-6
麻醉学对于我们环境的影响
据估计,现代医疗照护部门对美国 GHG 排放(这是医疗照护部门关注的、危害我们每个人的污染)的贡献率为 8.5%(图 1)。6,7 整个医疗照护部门都有必要评估并减少 GHG 排放对环境的影响。应对气候变化及其对下游人群健康的影响,是麻醉专业人士再次引领患者安全工作的机会。
医药相关性排放大约占到医疗照护部门 GHG 排放量的 20%,超过了食品、建筑或运输行业。6 挥发性麻醉剂和一氧化二氮所捕获的能量比二氧化碳多数百到数千倍。8 有一项研究发现麻醉气体可能在整个手术室 (OR) 的 GHG 排放中占 50% 以上。2
考虑到捕获能量的能力及其在临床上使用的浓度,地氟烷9 和一氧化二氮10 对环境造成的影响最大;七氟烷造成损害的程度最低,但其破坏力仍是强力 GHG 甲烷的数倍。在一项比较 GHG 排放量/最低肺泡 (MAC)-小时麻醉的研究中,丙泊酚对气候的影响比任何挥发性麻醉剂均低几个指数量级。8
最近的一项研究发现,院内输送的一氧化二氮危险且效率低下,超过 70% 的一氧化二氮会因泄漏而丢失,这加剧了一氧化二氮的危害。11 从医院存储到手术室使用这个过程中的一氧化二氮丢失是一个潜在的患者安全和职业安全问题。
对麻醉照护工作形式改变(图 2)可以提供机会来减少照护工作对环境的影响,同时也可为患者提供更有意义的医疗照护。例如,威斯康辛大学组建了一个包含麻醉专业人士在内的多学科手术室绿色团队,就麻醉气体废物产生率、低流量麻醉12 和挥发性麻醉剂的差异化 GHG 排放问题对麻醉科室进行了培训。在三年内,他们在每月节省 25,000 美元成本的同时,还将每个病例的 GHG 排放量减少了 64%。13 而且,即使卫生系统实施可持续的麻醉实践,如较低的新鲜气体流速 (<1 L/min)、可重复使用的仪器设备等,并消除了地氟烷和一氧化二氮的使用,在减少GHG排放方面麻醉专业人士仍有机会发挥重要作用。14
如何参与?
随着全球优先事项在转变,患者和医疗机构需要更具可持续性的经济模式,医疗照护应当作出改变。当前的医学实践是不可持续的。在一项史无前例的行动倡议中,包括《新英格兰医学杂志》和《柳叶刀》在内的 200 多种医学杂志发表了一篇述评,呼吁紧急行动起来,以减少温室气体排放,并防止其在日后带来危害。4,15
气候变化和环境污染是业已存在的全球性问题,可能引发焦虑和担忧,需要采取协调一致的集体行动来加以应对。16 很多大规模组织机构正在领导医疗照护行业应对气候危机:Healthcare without Harm、Practice Green Health 和 The Medical Society Consortium on Climate and Health。这三家组织机构正领导着美国可持续健康运动。此外,还有“Medical Students for a Sustainable Future”组织,这是学生领导的气候与健康倡导组织。
要成为可持续医疗照护的领导者,麻醉专业人士必须减少其自身工作导致的危害,并开始倡导建立更具可持续性的医疗系统。医疗照护机构具有极大的职责和能力。医疗照护部门创造了美国六分之一的 GDP;构建可持续的医疗照护系统17 可促进整个美国经济的变化。麻醉专业人士有机会来减少我们的工作造成的环境危害,并在下一次患者安全运动中担负起领导责任。
