OBJECTIVES
An increasing proportion of surgical and diagnostic and therapeutic procedures occur outside of traditional operating room suites. Several studies document the increased risks associated with caring for patients undergoing non-operating room anesthesia (NORA). NORA settings range from isolated areas of accredited hospitals to accredited ambulatory surgical centers to free-standing specialty procedural and diagnostic centers to accredited and non-accredited offices in which procedures are performed on anesthetized patients.
The 2022 APSF Stoelting Conference will address the unique challenges associated with providing safe care to patients in NORA environments. The conference will draw participants from the anesthesia professions, multiple additional fields in healthcare, industry, regulatory agencies, and healthcare administration. The goal of the conference is to develop multi-disciplinary, consensus-derived recommendations that the APSF and other organizations can use to advocate for changes that improve patient safety in NORA settings.
The objectives of this meeting are to:
- Understand the issues that may lead to mismatches in patient selection and preparation and the capabilities of NORA locations and their staff
- Review patient selection criteria, appropriate kinds and numbers of staffing, equipment and monitoring availability to deliver anesthesia appropriate to the situation and any other issues associated with potential patient safety problems in isolated procedure rooms, free-standing surgical centers, offices and procedure centers
- Discuss opportunities to promote patient safety using clear outcome measurements and data-driven improvement initiatives in all NORA cases
- Craft specific recommendations that APSF can use to influence changes that improve patient safety in NORA practices

Daniel J. Cole, M.D. is the President of Anesthesia Patient Safety Foundation and serves on the Board of Directors. A neuroanesthesiologist by training he currently services as a Professor of Clinical Anesthesiology in the Department of Anesthesiology and Perioperative Medicine at the David Geffen School of Medicine, University of California, Los Angeles. He has published extensively, with more than 350 original manuscripts, chapters, abstracts, and editorials to his credit. He is also a distinguished lecturer with over 300 invited presentations on a broad range of topics in anesthesiology.
Cole is a past president of the American Society of Anesthesiologists. He is the Executive Director for Professional Affairs for the American Board of Anesthesiology, and the American Board of Medical Specialties. Most importantly he is a champion for patient safety throughout his many roles in organized medicine.

Emily is passionate about patient safety, quality improvement, cardiac anesthesia, and gender equity in medicine, and has written about and spoken on these topics at the national level. She has a particular interest in patient safety and communication as well as non-operating room anesthesia. Brent is the Director of Advanced Practice, Chief CRNA, and Assistant Professor at Vanderbilt University Medical Center in Nashville, Tennessee. Brent has received numerous awards, among them the Agatha Hodgins Award for outstanding nurse anesthesia student, the Pennsylvania Association of Nurse Anesthetists’ Didactic Instructor of the Year Award, the University of Pittsburgh School of Nursing’s Outstanding Young Alumnus Award, and the Dr. Nevin Downs Leadership Award. Dr. Dunworth is a certified nurse executive, a fellow of the American Association of Nurse Anesthetists, and has lectured nationally on a variety of topics in nurse anesthesia and maintains clinical practice at Vanderbilt.

Michael L. Kochman, MD, AGAF, MASGE is the Wilmott Family Professor of Medicine and Professor of Medicine in Surgery in the Gastroenterology Division at the Perelman School of Medicine at the University of Pennsylvania and Hospital of the University of Pennsylvania. Dr. Kochman is a graduate of Northwestern University (BA, Economics, 1982) and the University of Illinois Medical School at Chicago (MD, 1986). He served his residency at the University of Illinois at Chicago and was Chief Resident at West Side VA under Dr. Clifford Pilz from 1989-90. Subsequently Dr. Kochman completed his fellowship in Gastroenterology and Hepatology at the University of Michigan Hospitals and Clinics in Ann Arbor in 1993 under the direction of Drs. Chung Owyang and Tadataka Yamada. During that time Dr. Kochman also underwent training in interventional endoscopy and had specialized training in endoscopic ultrasound at Indiana University under the auspices of Drs. Robert Hawes and Maurits Wiersema. Subsequently he was recruited to the University of Pennsylvania by Dr. Peter Traber, were he rose through the ranks from Assistant Professor.
Dr. Kochman is Editor for Techniques and Innovation in Gastrointestinal Endoscopy. Dr. Kochman has published over 250 articles, chapters and videos. He has edited 19 published books: among them, the Clinicians Guide to Gastrointestinal Oncology (ed Kochman), Advanced Gastrointestinal Endoscopy (eds Ginsberg, Gostout, Kochman and Norton), and Endoscopic Oncology: Gastrointestinal Endoscopy and Cancer Management (eds Faigel and Kochman).
While at the University of Pennsylvania he has served in various capacities including as Vice-Chair for Clinical Services and as Development Officer for the Department of Medicine, on the School of Medicine Committee of Appointments and Promotions (COAP), the Physicians Billing Oversight Committee and numerous Departmental Review and Search Committees. Within the GI Division he has served as Fellowship Chairman and runs the regular endoscopy and advanced endoscopy training programs and directs the Wilmott Center for Endoscopic Innovation, Research, and Training. He has received a number of awards including the Luigi Mastroianni Clinical Innovator and Special Recognition Patient Advocacy Awards from the Health System as well as the Sid Cohen, MD and Donald Martin, MD awards for the education of fellows and residents.
Dr. Kochman has served many local and national societies in a variety of positions. He was honored and inducted as a Fellow into the College of Physicians of Philadelphia. The ASGE recognized him as the ‘Master Endoscopist’ in 2015 and the AGA recognized him as the ‘Distinguished Academic Clinician’ in 2018. His current major national commitments are to the American Gastroenterological Association (AGA) as a Governing Board Councilor, the Society of American Gastrointestinal and Endoscopic Surgeons (SAGES) and to the Natural Orifice Surgery Consortium for Assessment and Research (NOSCAR). He has served the AGA on the Education Committee, the Public Affairs Committee, was Chair of the Regulatory Workgroup and is past Chair of the Center for GI Technology. Dr. Kochman served as Chair of the GIE Editorial Board, treasurer of the ASGE Foundation, and was a Councilor of the ASGE Governing Board. Also, for the ASGE Dr. Kochman has served on the Post-Graduate Education Committee, the EUS SIG, the Research Committee, the Program Committees and has directed numerous post-graduate courses. He is a past president of the Delaware Society for Gastrointestinal Endoscopy and was the Program Chairman for the Pennsylvania State Gastroenterology Society.
Dr. Kochman regularly speaks for various local GI societies and at SGNA meetings. He is frequently a faculty member at national and international meetings as a lecturer, moderator and at Meet the Professor sessions on various topics including management of neoplasia, maintenance of luminal patency, EMR, EUS, and ERCP. He also serves as faculty for hands-on training courses held both nationally and internationally for EUS, ERCP, endoprosthetics, and hemostasis.

Dr. Thomas Farrell is a fellowship trained interventional radiologist at NorthShore University HealthSystem, Illinois where he has practiced for the past 20 years. He performs the full range of interventional radiology procedures and has an excellent working relationship with his anesthesiology colleagues. He is also a Fellow of the Society of Interventional Radiology and serves as an examiner for the American Board of Radiology.

Sarah K. Gualano, MD, MBA, is a Clinical Associate Professor in the Division of Cardiovascular Medicine, Associate Vice Chair for Diversity, Equity and Well-being for the Department of Internal Medicine at the University of Michigan. Dr. Gualano also serves as Associate Section Chief of Cardiology at the LTC Charles S. Kettles VA Ann Arbor Health System. Dr. Gualano was recently selected to participate in the prestigious Rudi Ansbacher Women in Academic Medicine Leadership Scholars Program. Dr. Gualano’s clinical interests are coronary and structural heart disease, with a focus on regional care for the veteran population. Outside the cath lab, her administrative and academic interests include gender equity in medicine and physician well-being, and their relationship to the clinical enterprise at an academic medical center.

Dr. Sweitzer is an author and the editor of Preoperative Assessment and Management (currently 3rd edition). She has authored over 100 peer-reviewed manuscripts and abstracts and over 30 book chapters. She lectures nationally and internationally on preoperative medicine and ambulatory surgery topics. She is the Executive Editor of A&A Practice, and on the editorial board of Anesthesiology and Anesthesia and Analgesia. She hosts a monthly podcast for Anesthesiology, the premier journal of the American Society of Anesthesiologists. Dr. Sweitzer is President of the Society of Ambulatory Anesthesia (SAMBA). She is a Fellow of the American College of Physicians, the American Society of Anesthesiologists and SAMBA. Fred E. Shapiro, D.O., FASA is an Associate Professor of Anesthesia, Harvard Medical School, Massachusetts Eye and Ear (MEE) Boston, MA. He has extensive research publication experience and a reputation nationally and internationally in the field of patient safety in anesthesia and surgery performed outside the operating room.
He created the first Harvard Medical School Office- based anesthesia CME Course, “The Manual of Office Based Anesthesia Procedures”, presented at Harvard Medical School, led to inception of an OBA curriculum, and incorporated into the Society for Ambulatory Anesthesia (SAMBA) national program. He wrote the seminal ASA CME article on Office Based Anesthesia: Safety and Outcomes.
He participated in The MA Board of Medicine task force to define qualifications for practitioners supervising office-based procedures and surgery, recently revised the Massachusetts Medical Society (MMS) Office Based Surgery Guidelines and act as a resource to ASA governance regarding similar statewide national legislative changes.
In 2009, he co-founded The Institute for Safety in Office Based Surgery (www.ISOBS.org), an independent, multidisciplinary, non-profit organization to promote patient safety in office based surgery through scholarship, patient and physician education.
He is on the SAMBA Board of Directors, Chair of SAMBA OBA Committee, and a member of the APSF Patient Safety Advisory Committee on Non-Operating Room Anesthesia.

Rita Agarwal, MD is a Clinical Professor of Anesthesiology at Stanford University School of Medicine; she has been involved with anesthesia, in particular pediatric anesthesia education, for over 20 years. She served as the Pediatric Anesthesia Program Director for 17 years at Children’s Hospital Colorado, and is now the Chair of the Pediatric Education Committee at Stanford. Her other passions include pediatric pain management, and she has served as the Director of Acute Pain Services at Children’s Hospital Colorado. She is now an attending anesthesiologist and inpatient pain management attending at Lucille Packard Children’s Hospital. Improving patient safety by enhancing communication, collaboration, and team building are primary interests for Agarwal. She is a senior editor for the American Board of Anesthesiology certification examination and a junior editor for the Pediatric Anesthesiology certification examination. Patty Mullen Reilly is a CRNA at Chester County Hospital, Penn Medicine in West Chester, Pa. She is also a part of the Clinical Research and Medical Science team within the Patient Monitoring and Respiratory Interventions businesses for Medtronic which is located in Boulder, CO. Patty has been involved in Patient Safety for the past 25 years as a part of the Corporate Advisory Board and the Committee on Technology of the APSF and most recently as a part of the Board of Directors for the APSF. Her safety work has included work with the Lucien Leape Institute as well as membership on the Patient Safety Coalition of the NPSF/IHI. Her corporate work has included ongoing education about the important work of the APSF and the NPSF/IHI and the impact on patient care. Most recently Patty has obtained her CPPS credential, becoming a Certified Professional in Patient Safety. Patty lives in West Chester, Pa with her husband and daughter.

Dr. Wanderer is a Professor of Anesthesiology and Biomedical Informatics. He is focused on applying clinical informatics to advance medicine, by leveraging the electronic health record, clinical decision support, and data science.
He chairs Vanderbilt’s Periprocedural Informatics Governance Committee, is the Associate Fellowship Director for the Clinical Informatics Fellowship Program and co-course director for the Foundations of Clinical Informatics in the Masters of Applied Clinical Informatics Program. He leads Vanderbilt’s Physician Builder program and is the Associate Medical Director for both the Vanderbilt Anesthesiology and Perioperative Informatics Research (VAPIR) Division and Vanderbilt’s Preoperative Evaluation Center.
Nationally, he serves the Anesthesia Quality Institute as Vice Chair for the Registry Operations and Data Definitions Committees, works as an Oral Boards Examiner for the American Board of Anesthesiology, and is on the Board of Directors for the Society for Technology in Anesthesia. He has been thrice elected to Epic’s Anesthesiology Specialty Steering Board and co-chairs the anesthesia mobile technology subgroup.

Girish P. Joshi, MBBS, MD, FCAI is a Professor of Anesthesiology and Pain Management at the University of Texas Southwestern Medical School in Dallas, Texas. He is an attending anesthesiologist at the Parkland Health and Hospital System, Dallas, Texas. Dr. Joshi received his medical degree (Bachelor of Medicine and Bachelor of Surgery, M.B.B.S.) and Doctor of Medicine (M.D.) in anesthesiology from the B. J. Medical College at the University of Pune, India. Elizabeth Rebello, MD, FASA, CPPS, is a practicing anesthesiologist and Professor of Anesthesiology and Perioperative Medicine at the University of Texas MD Anderson Cancer Center. A pharmacist by training, she has been involved in quality and patient safety efforts in the perioperative environment and Diversity, Equity, and Inclusion efforts for over fifteen years.
She serves as the Vice-Chair of the American Society of Anesthesiologists’ (ASA) Quality, Management and Data Administration Committee and led the Medication Safety Workgroup to standardize drug concentrations. Jeffrey B. Cooper, Ph.D. is Professor of Anaesthesia at Harvard Medical School and Massachusetts General Hospital. He is the founder, Executive Director Emeritus and Senior Fellow of the Center for Medical Simulation, which is dedicated to the use of simulation in healthcare to improve the process of education and training and to avoid risk to patients.
Cooper is one of the pioneers in what is now called patient safety. He did landmark research in medical errors in the 1970’s, is a co-founder and on the Board of Advisors of the National Patient Safety Foundation. He was Associate Director and later Director of Biomedical Engineering at the Massachusetts General Hospital for ten years and Director of Biomedical Engineering of the Partners Healthcare System for fourteen years. He is author or co-author of over 130 peer reviewed articles and book chapters.
Cooper has been awarded several honors for his work in patient safety, including the 2003 John M. Eisenberg Award for Lifetime Achievement in Patient Safety from the National Quality Forum and the Joint Commission on Accreditation of Healthcare Organizations and the 2004 Lifetime Achievement Award from the American Academy of Clinical Engineering. The Department of Anesthesia and Critical Care of the MGH established the Jeffrey B. Cooper Patient Safety award in his honor. He received the Distinguished Service Award of the American Society of Anesthesiologists in 2013, the first non-MD to receive the honor. In 2014, he was awarded the JS Gravenstein Award for lifetime achievement by the Society for Technology in Anesthesia and named one of the first two members for the Hall of Fame of American College of Clinical Engineering and is among the inaugural fellows of the Academy of the Society for Simulation in Healthcare.

Amy Vinson is a Pediatrician and Pediatric Anesthesiologist at Boston Children’s Hospital and is an Assistant Professor of Anesthesia at Harvard Medical School. She attended college and medical school in Georgia before coming to New England where she completed a residency in pediatrics at Brown, an anesthesiology residency at BIDMC and Pediatric Anesthesiology fellowship at Boston Children’s Hospital, where she served as chief fellow. She has a long standing interest in wellness, burnout, peer support, and substance use disorder in anesthesiologists and has lectured widely on these subjects. She established wellness programs at Beth Israel Deaconess Medical Center and Boston Children’s Hospital and has consulted on the formation of many other programs throughout the country. She currently serves as the Chair of the ASA’s Committee on Physician Well-being, is the ASA representative to the National Academy of Medicine’s Clinician Well-being Collaborative, and is the Director of Clinician Well-being in her own department. Her research on Burnout in Anesthesiologists – the largest such study of anesthesiologists to date, has just been published in Anesthesiology and she has numerous other publications and research manuscripts on subjects related to overall physician well-being. She is a self-described “well-being pragmatist” focusing on systemic and organizational factors impacting well-being and burnout. To stay balanced, she enjoys time with family, making music, fitness and home improvement projects.

Dr. Mahajan serves as the Peter and Eva Safar Professor and Chairman of University of Pittsburgh Medical Center and Health System. He is a Professor of Anesthesiology and Bioengineering at the University of Pittsburgh. He is also the Director of Perioperative and Surgical Services and serves as the Senior Advisor to UPMC Enterprises.
Dr. Mahajan received his medical specialty training in anesthesiology and cardiac anesthesiology, as well as a PhD in Physiology with a focus in the field of cardiac electrophysiology at UCLA. Dr. Mahajan is funded by the NIH for his research in translational and clinical medicine. He has published more than 175 peer reviewed papers in his career and as an editorial board member and reviewer of several scientific journals he actively contributes towards the research and innovation in the field. Piyush Mathur, MD, FCCM is a Staff Anesthesiologist and Critical Care physician at Cleveland Clinic, Ohio with more than 20 years of clinician experience. He has also served in the role of the Quality Improvement Officer and chair, compliance committee, Anesthesiology Institute since 2011. He completed his medical school training in India from Armed Forces Medical College (AFMC, Pune) followed by residency in Anesthesiology and fellowship in Critical Care from Cleveland Clinic. He subsequently completed a one year, quality fellowship at Cleveland Clinic and training in computer programming, machine learning and deep learning. He is the past program director of the anesthesiology critical care fellowship program at Cleveland Clinic.

He has focused his research on applications of artificial intelligence (AI) in healthcare for the last many years. He is a leader in quality and patient safety who has innovated and successfully implemented many algorithms and tools in electronic health records such as difficult airway identification (EPIC), anesthesia awareness alert (DSS, Talis), antibiotic alert (ACG, Talis). Recipient of multiple innovation awards at Cleveland Clinic, he is leading innovation efforts in integrating machine learning and artificial intelligence in healthcare. Current, projects include AIDE (Artificial Intelligence Diagnosis Engine), SALUS (robotic artificial intelligence patient safety system), BRAINS (Biologically Relational Artificial Intelligence Networking System). Dr. Mack serves as the Vice Chair for Patient Safety and Quality Improvement at Weill Cornell Medicine / New York-Presbyterian Hospital. In the past she has served as Director of Neuroanesthesiology and Director of Non-Operating Room Anesthesiology. In addition to establishing education programs related to Neuroanesthesiology and NORA, she has vast experience in working with architects and operations leaders regarding the construction and programmatic development of INR, IR, GI and catheterization and ECT suites as well as hybrid operating rooms. She is a fierce advocate for both patient and clinician safety in these areas and has worked to align procedural areas with all standards of perioperative care. She has developed a didactic and experiential Quality Improvement educational program for trainees. She led the successful PBLD program of the PGA for 15 years and has served as a national and international speaker on topics of Neuroanesthesiology, NORA and Patient Safety and Quality Improvement. She currently serves as Chair of the NYSSA Committee on Patient Safety and Quality Improvement and is honored to serve on the APSF Patient Safety Priority Advisory Group for NORA.

John “JW” Beard, MD is the Chief Medical Officer of GE HealthCare – Patient Care Solutions (PCS). JW is responsible for contributing clinical expertise to strategic business processes including innovation, clinical evidence generation, health economics and outcomes research, and scientific communication. He brings the voice of the patient and clinician to all levels of the organization and represents PCS externally as the primary medical executive. JW was previously at ICU Medical Inc., where most recently he served as Chief Medical Officer. Prior to joining GE, Beard was in clinical practice for fifteen years, including roles in pre-admission clinics, operating rooms, and labor and delivery units. During his practice, Beard led multiple quality improvement initiatives and held leadership positions including Chairman and Medical Director of the Department of Anesthesia.
